REQUESTING A SECOND FSA OR HSA DEBIT CARD

Participants are able to request a secondary card. Initially, participants are provided one card however they have the availability to request a secondary card in the name of one of their dependents.

This process just recently changed when we transitioned to the new benefit card. Requesting a second card simply requires the Secondary Benefits Card Authorization Form be completed and returned to participantservices@ebcflex.com or by logging into your employee portal at www.ebcflex.com. There is no fee for the secondary card.

2017 ANNUAL BENEFITS ENROLLMENT

The annual enrollment for 2017 benefits is closed. If you did not visit Myjob to enter changes to your 2017 benefits, 2016 health, dental and/or vision coverage will be continued for 2017. However, any FSA coverage will be discontinued as of January 1, 2017.

Benefit elections and/or modifications made during the open enrollment period will be effective January 1, 2017, and will remain in effect for the entire calendar year (January 1 through December 31). Mid-year changes are not allowed unless you experience a qualified life event and provide timely notification to HR-Benefits (within 30 calendar days of the event).

BIWEEKLY PAYROLL - BENEFIT DEDUCTIONS FOR JANUARY 2017

The first pay date in 2017 for biweekly paid staff is January 11, 2017. Because the pay period for the January 11th check is entirely in 2016 (12/18-12/31/16) there are some things to be aware of regarding your benefit deductions on that check.

Your benefits payroll deductions will reflect the 2017 rates starting on the January 11th check UNLESS YOU MADE CHANGES TO YOUR MEDICAL, DENTAL OR VISION ELECTION FOR 2017. If you changed plans (e.g. EPO to HDHP) or enrolled in a benefit for the first time, you will NOT have benefits deductions taken on the January 11th check. You will have a DOUBLE DEDUCTION taken on the January 25th check, so please plan accordingly.

The Aurora Employee Assistance Program (EAP) is a benefit for all Marquette University employees, spouses and dependents. The EAP provides work-life services, short-term counseling, referrals and resource identification. Services provided are kept confidential.
